Aplicaciones clínicas, limitaciones y valor pronóstico del análisis de bioimpedancia eléctrica en el paciente crítico: Revisión narrativa

Resumen

El paciente crítico presenta alteraciones drásticas en la composición corporal, haciendo crucial su monitorización. Esta revisión narrativa resume la literatura sobre las aplicaciones clínicas, las limitaciones y el valor pronóstico del análisis de bioimpedancia eléctrica (BIA) en cuidados intensivos.

Se realizó una búsqueda sistemática en PubMed, Cochrane Library y Google Scholar utilizando términos MeSH y palabras clave relacionadas con BIA, composición corporal, estado nutricional y enfermedad crítica. Se seleccionaron artículos científicos y capítulos de libros pertinentes a los subtemas de investigación.

Debido a la reanimación hídrica, las ecuaciones predictivas tradicionales de la BIA sobreestiman la masa libre de grasa. La evidencia actual justifica emplear modelos de ajuste por sobrehidratación y priorizar parámetros celulares directos, como el análisis vectorial y el ángulo de fase (AF). El AF es un biomarcador subrogado útil para detectar el deterioro de la integridad celular y pérdida muscular. Su monitorización longitudinal tiene un alto valor pronóstico: un AF disminuido predice mayor mortalidad, ventilación prolongada y secuelas neuromusculares del síndrome post-UCI (debilidad adquirida en la UCI y disfagia post-extubación).

Interpretada con rigor metodológico, la BIA identifica la sobrehidratación, estratifica el riesgo de complicaciones y diagnostica fidedignamente las reservas corporales. Tras el egreso de la UCI, permite evaluar objetivamente la recuperación a largo plazo.
